In order to issue invitation letters, you will need to provide the following information:
- Surname (Family name)
- First Name
- Date and Place of Birth
- Passport Number
- Nationality
- Country of Issue
- Expiration Date
- Affiliation
- Dates of arrival and departure
- Hotel/Lodging information
Because this event is subordinate to the SKA Engineering Meeting 2016, you will need to register to that meeting, and have your invite issued through the LOC for the SKA Engineering Meeting 2016.
